Kishan G. Patel, MD, shares a case of sutureless intrascleral fixation of a 3-piece IOL (CT Lucia 602, Carl Zeiss Meditec) using bent 27-gauge needles. The patient has a history of a dislocated crystalline lens and underwent previous vitrectomy and lensectomy and was subsequently left aphakic. One haptic is externalized at a time and low-temperature cautery was used to create haptic flanges. This technique also shows the use of the corneal light reflex to help align the scleral tunnels and center the IOL, rather than using a toric marker.
