Rhegmatogenous Retinal Detachment With Posterior PVR

This video features a male patient with a history of vitrectomy for epiretinal membrane who presented with hand movement visual acuity 3 months after the previous surgery. Soon after the previous vitrectomy, the patient also underwent cataract surgery with posterior capsule rupture resulting in a mildly dislocated PCIOL. Dr. Moraru performs 25g pars plana vitrectomy and, with the help of triamcinolone and blue dye, noticed that the posterior PVR was due to large posterior hyaloid remnants and unpeeled ILM from the previous surgery. The video highlights the importance of complete removal of posterior hyaloid and an ILM peeling technique on detached retina and macula by double-staining and by gentle pinch-and-peel maneuvers.
