In this case, a 79-year-old patient presented with a 2-week-old retinal detachment. Martín Charles, MD, demonstrates his approach to this case, with his first time using the Unity Vitreoretinal Cataract System (Alcon) platform. He performs a four-port vitrectomy, connecting the chandelier at the 12 o’clock position, and centering the visualization system with Ngenuity (Alcon) to begin with the core vitrectomy.
Dr. Charles uses the maximum cut speed of 30,000 cuts per minute with the peristaltic pump in Flow Control mode. Once the central vitreous is removed, he proceeds to release the vitreous traction over the retinal tears. He injects perfluorocarbon liquid to stabilize the posterior pole and continue with the vitrectomy. He comments on the platform's stability. With maximum cut speed and considerable flow, he can work with safety, performing meticulous shaving of the vitreous base and of the adhesions around the tear.
