Tian Bei, MD, PhD, demonstrates a petaloid self-coiling internal limiting membrane (ILM) covering technique for macular hole repair. Multiple petaloid ILM flaps are created around the macular hole and allowed to self-coil toward the center, gently covering the defect without aggressive tucking or repeated manipulation of the foveal floor and retinal pigment epithelium. In this selected case, air tamponade was used, and postoperative OCT showed early anatomical closure. The technique emphasizes a gentle, RPE-sparing approach to macular reconstruction in select cases.
