Smaller-gauge vitrectomy systems have influenced the multifunctional use of vitreous cutters, such as scissors, picks, spatulas, and backflush cannulas. They provide fewer instrument changes and less entry and exit to the eye, and offer features that facilitate and simplify the surgery. In this video we are presenting unimanual and bimanual approaches with 27-gauge vitrectomy in combined tractional-rhegmatogeneous retinal detachment. The need for instrument exchange is decreased even when peeling the membrane from the mobile surface of the combined tractional-regmatogenous retinal detachment.
