This video demonstrates a technique for fixation of the IOL-bag complex using capsular tension segments and double-flanged polypropylene sutures. A 7-year-old girl with autism spectrum disorder and a developmental delay presented with high anisometropia hyperopic astigmatism and spectacle noncompliance. Examination showed inferior crystalline lens coloboma in both eyes. Due to the spectacle noncompliance, emmetropia was targeted for IOL fixation.
