This video demonstrates the cap-to-flap technique employed for SMILE enhancement with the VisuMax (Carl Zeiss Meditec). Outside the United States, Circle Software (Carl Zeiss Meditec) facilitates the conversion of a preceding SMILE cap into a LASIK flap. The software is not, however, approved in the United States. Several off-label cap-to-flap (pseudo-Circle) techniques have therefore emerged. Our preferred technique involves the creation of a femtosecond lamellar ring that intersects with the SMILE sub-cap plane. Following an intentional suction break, the eye is redocked to complete a side cut. This method allows for intentional communication between interfaces with remarkable coherence.
