Murtaza Adam, MD, performed a hybrid 25- and 27-gauge pars plana vitrectomy on a monocular, phakic, type 1 diabetic 30-year-old presenting with 20/400 vision OD and a tractional retinal detachment. Using a 25-gauge cutter for the core vitrectomy, a 27-gauge cutter to dissect the fibrovascular plaque, multiple sessions of panretinal photocoagulation, and repeatedly injecting triamcinolone to identify all adherent vitreous strands, Dr. Adam removed all significant traction.
