This video presents a 27-gauge trocar-cannula based approach for intrascleral fixation of the dislocated three-piece intraocular lens. As a modification, we only created two sclerotomies during the procedure by using the same pars plicata sclerotomies for both haptic fixation and vitrectomy. The infusion cannula was also placed in the anterior chamber. Our aim was to develop a less invasive and simpler approach by minimizing the number of scleral wounds.

Would this work with 25 ga instrumentation? Or would the tunnel be too big? Just curious.
