This video demonstrates what is currently the smallest gauge system for macular hole surgery: 27 Gauge Transconjunctival System form D. O. R. C. After vertical insertion of the cannulas, Yusuke Oshima, MD, uses a wide angle viewing system with Xenon light Bright Star for visualization. The ILM is stained and peeled around the hole. The instrumentation is removed with no blood formation.

at this rate, the gauge will get so high, that soon, no instruments will even bother entering the eye.
