A 78-year old male patient presents is a history of gradual decrease in vision over 6 months. Repeated ultrasound imaging showed evidence of vitreous hemorrhage so a pars plana vitrectomy was performed. Vitrectomy did not improve visualization which led the authors to the conclusion that the issue was an opacified IOL.
