Anterior Tear

A modified dimple down technique confirms a capsular tag is present in this femtosecond laser created capsulotomy. Surgery proceeds with extreme caution to avoid enlarging a suspected anterior capsule tear. The tear ends up extending but the hole is small enough that an in-the-bag IOL is placed. Upon insertion, the IOL haptics are stuck together and one unfolds in the sulcus. Additional intraocular maneuvers are required to move the haptic into the bag cause the posterior capsule tear to expand.
