This patient underwent penetrating keratoplasty (PK) in the right eye for severe keratoconus, achieving a BCVA of 20/20 postoperatively. The left eye also progressed to severe keratoconus, with BCVA limited to 20/150 due to 10 diopters of astigmatism. Instead of PK, corneal tissue addition keratoplasty (CTAK) was recommended. Following CTAK, the UCVA in the left eye improved significantly to 20/50 at 1 month postoperatively. The patient was then referred to an optometrist for further enhancement of vision with corrective glasses.
