iTrack Advance: Troubleshooting Canal Blockage

This surgical video demonstrates a routine canaloplasty procedure complicated by a blockage within the canal. The procedure was smooth until approximately nine clock hours into the advancement of the device. At this point, there is notable resistance to further catheter delivery. A few clicks of viscoelastic from the device can sometimes free the canal from blockage. Unfortunately it was not able to open the canal in this particular case. Retraction of the catheter was performed and canaloplasty was successfully performed in 270 degrees of the angle. A decision was made not to cannulate backhand to attempt canaloplasty of the other 90 degrees. The iTrack Advance (Nova Eye Medical) system and the illuminated fiber optic tip are helpful in determining whether or not the catheter is in its intended location. It can help in detecting inadvertent catheterization of collector channels, the supraciliary space, and confirming blockages within the canal. The soft nature of the catheter also reduces the potential trauma that could be inflicted when the prior circumstances arise.
