Majid Moshirfar, MD, presents a case of pupilloplasty in a patient with a past history of ICL removal during cataract surgery, complicated by traumatic iris defects and an irregular pupil. The patient developed significant photophobia and a visual disturbance due to transillumination defects and poor pupillary function. In the setting of a multifocal IOL, pupilloplasty was performed to reconstruct the pupil, reduce light scatter, and restore a more functional and symmetric iris.
