E-DMEK Fuch's Dystrophy with Cataract

Show Description +

In this video, I demonstrate my technique of E-DMEK or Endoilluminator assisted DMEK. Here, the endoilluminator is used to enhance visualization and 3-dimensional perception of the DMEK graft. This helps the surgeon greatly to comprehend more easily graft position, orientation, morphology and dynamics, all of which leads to easier and faster surgery. It avoids unnecessary maneuvers that might otherwise be required to analyze graft position and thereby decreases potential graft damage. It makes determining graft orientation with respect to endothelial versus Descemetic side a non-touch technique. Light from the endoilluminator is shown obliquely while the microscope light is switched off. This light bouncing off the folds and edges of the DMEK graft allows very good 3-dimensional perception even in a hazy, edematous cornea.

Dear Dr. Siepser, Thank you very much for your comment. Its an honour coming from you. I generally avoid back scatter by holding the probe at a slight distance from the cornea (not touching it) and by adjusting the angle, however your instrument sounds wonderful and I would definitely love to use it!! Dear Dr. Jacob, Excellent surgery. Might I recommend the Siepser Shielded Probe that was presented in OSN many years ago for seeing anterior chamber membranes and vitreous and consistently used for DMEK and DSEK, see "Double Barreled DSAEK in OSN. This light probe is specifically designed for this application and has a hood to diminish back scatter. This product is available from Escalon Medical, 2440 South 179th Street, New Berlin, WI 53146 USA tel: 800-486-4848 It is the TR9879-11 Siepser Shielded Probe. Please let me know how it works in your hands.
