Large corneal dermoids require lamellar keratoplasty to avoid loss of corneal transparency. Girija Devi shows a deep anterior lamellar keratoplasty (DALK) performed for a large corneal dermoid covering nearly 4/5th of the cornea in a 14-year-old boy. Nystagmus and amblyopia reduced vision to hand movements.
DALK by big bubble technique is not possible in such a structurally altered cornea. First, the edges of the dermoid are marked with 8.5 trephine, then the dermoid is removed. The underlying opaque cornea was dissected layer by layer until the transparent deeper layer was reached. Descemet's membrane was stripped from the donor corneal button and sutured to the recipient bed with interrupted 10-0 sutures. The postoperative picture shows a clear graft with good cosmetic results.
