Remin Nath, FRCOphth, and Ahmed El-Amir, FRCOphth, demonstrate an alternative strategy for implanting an intraocular lens in the anterior chamber by a pars plana approach. The patient had previously undergone retinal detachment surgery with silicone oil. An ARTISAN Phakic IOL (Ophtec) is placed in the anterior chamber. Attempts to enclavate the iris between the fixation arms prove unsuccessful. The technique of iris enclavation with standard methods to insert an ARTISAN lens in the anterior chamber can be challenging, particularly in the presence of a soft, vitrectomized eye. Using the 23-gauge ports, iris enclavation is achieved using a hydrodissection cannula passed through the pars plana.
