The results indicate that the non valved implant is less efficacious than the valved implant. Why then use a less efficacious implant at all? It is always better to try and make the initial surgery as successful as possible. I have always suggested that the cytokine levels in primary aqueous are the cause for greater failure in the Ahmed implant. May I suggest that if one chooses to use the Ahmed then lower the pre-operative pressure for a number of days prior to the surgery to decrease the levels of TGFbeta, and thus increase the chances of less fibrosis in the implant. I have advocated the use of a supra-Tenon implant when the initial implant fails, and this can be done with a 250 Baerveldt or Molteno3SS very easily. Jeff Freedman
