Very Long Standing Dense Post Rubella Cataract

A middle-aged male patient presents with poor vision in one eye due to a post rubella cataract with an extremely dense nucleus. After successful emulsification of 80% of the nucleus, a thick vitreous strand, protruding through the zonules appears in the anterior chamber, necessitating bimanual 2 port anterior vitrectomy. After resuming phacoemulsification the cornea wrinkles on occlusion break, indicating a significant surge. Dr. Khiun Tjia explains the cause of this surge and successfully manages to remove vitreous and remaining lens fragments.
