A 57-year-old diabetic was referred for a tractional retinal detachment after a failed vitrectomy. Her vision was light perception with faulty projection. The IOP was 42 with rubeosis and small pupil. Due to the previous failed vitrectomy and a dense cataract a decision was made to perform scleral buckle, phacoemulsification with IOL implantation, and vitrectomy.
