Alin Stefanescu-Dima, MD, PhD, presents the surgical challenges of traumatic cataract, and innovative ways to overcome them. A 62-year-old, Caucasian female suffered a blunt ocular trauma 20 years ago, after which she developed a traumatic cataract but retained useful vision. During the last year, she complained of progressive visual deterioration and a clinical exam revealed a corticonuclear cataract, iridophacodonesis, and zonular dehiscence of more than 180 degrees.

Salutare, da,tot prin Frantzuzia, ca si ultima data cand ne-am vazut...M-am apucat de plastica intre timp ( nici mie nu mi-a venit sa cred ce alegere am facut :-), dar se castiga mai bine cu mai putin stres, si ma cam saturasem de genul de operatii cu inele pe care asa de frumos l-ai rezolvat ( dar tb sa recunosti ca mai lasi ceva coronare prin sala la sfarsit ). Eu sincer ma bucur ca toti am reusit ( asa cum am putut si noi, neajutati de nimeni ) sa ne dezvoltam tehnici chirurgicale si cariere asa cum ne-am dorit. Inca o data, felicitari si spor in continuare!

Very good observation, Cionni ring is a good alternative, but at that moment it was not available in our country. Regarding the Ahmed segment, it is my opinion that used alone, it provides good vertical support, but it should be employed in conjunction with a regular CTR in order to obtain horizontal support for the lens bag equator, especially during the phaco procedure, but also postoporatively.

The first & second CTR could have not been used, instead a single Cionni Ring may have done the job better. Such a large zonular dehiscence may also be managed with an Ahmed segment, it produces least stress on remaining zonules.
