In this challenging case, three surgeries were performed in one session by Enver Mirza, MD. In the case, a 33-year-old male patient had blunt trauma to his right eye due to a firework explosion. On examination, an angle recession close to 360 degrees was observed, and the trabecular meshwork had heavy pigmentation compared to the fellow eye. The patient, who was started on maximum medical glaucoma treatment, experienced a decrease in visual acuity 2 months after treatment, photophobia, elevated IOP, and development of an allergy to topical glaucoma medication. Therefore, gonioscopy-assisted transluminal trabeculectomy (GATT) combined with phacoemulsification and a pupillary cerclage technique were performed.
