Titanium Macular Buckle Placement

Levent Akduman, MD, FASRS, demonstrates the placement of a titanium macular buckle in the eye of a patient with high myopia, a posterior pole detachment, and maculoschisis which decreased vision to hand motion. The patient was phakic with a moderate cataract. The superotemporal quadrant was exposed, a 5-0 mersilene suture was passed with bites 4.5 mm apart at 20 mm from the limbus perpendicularly. IOP was reduced with a paracentesis and the implant was placed. Surgeons observed the indentation of the implant inside the eye using a chandelier light and a wide-angle viewing microscope. No vitrectomy was performed. A slight adjustment was made while visualizing the indentation, and surgeons placed one 5-0 mersilene suture in one of the anterior horns of the implant to stabilize it. Six months after surgery, the posterior pole retinal detachment was resolved and maculoschisis was totally gone with only the external indentation. The patient’s vision was improved to 20/100 despite the cataract present.
