In this case, a patient with a history cataract surgery and single-piece IOL implantation presented with a dislocated IOL moving freely in the anterior chamber. Ozana Moraru, MD, details her plan to remove the IOL and to implant a new three-piece IOL more suitable for the sulcus. Complete removal of the thick cortex remnants from the bag periphery and a posterior capsule polishing were also performed. The new IOL was implanted in the bag, in a position perpendicular to the area of the previous anterior capsulotomy run-out.
