The surgeon dissected from the temporal side of the flap, bringing the spatula towards the hinge. As the hinge was dissected (from temporal to nasal), the surgeon lifted the elbow of the spatula, placing undue pressure on the hinge. This resulted in a tear that extended nasally.
In response, the surgeon decided to continue the dissection, sparing the nasal flap side cut to use it as the new hinge. The flap is reflected nasally and gently pulled to allow for sufficient stromal bed exposure. Myopic treatment is performed successfully and the flap is repositioned without incident.
