I have done plenty of these small pupil Flomax cases without the Malyugin ring. Typically, even though successful, I still regret not placing the Malyugin ring at the start. I tell myself, when in doubt, use it....but when that pupil is 5mm or so you think "it's dilated enough." However, toward the end of the case when it is 2-3 mm you realize how much better life would have been with the ring. I cannot recall a single case where I regretted using the ring. Any iris/pupil abnormalities from the Malyugin ring are only noticed at the slit lamp and not by the patient or any other observer.

lol. watch me now, as i do it with one hand tied behind my back! great skill- don't get me wrong- but one can't possibly think this is a good idea for the vast majority of us. there is a reason why small pupil cases have higher risk of complications, and we ought to stretch or use the ring- the latter has almost zero cosmetic/functional effect, and the former- very low rate of untoward sequeale.

by the way, i do see a small focus of damage to the iris sphincter where it was grabbed by the phaco needle....
