Arun Gulani, MD, presents a case of pseudophakic bullous keratopathy using his "Keyhole" transplant technique. First, the surgeon debrides the epithelium for visualization. A modified Sinskey hook is used to score Descemet's membrane and scroll it for removal through the paracenthesis. After the donor graft is inserted though a scleral tunnel, the anterior chamber is filled with air to position the graft.
