Scleral Flap Retraction Suture for Sclerotomy During Trabeculectomy

Zia ul Mazhry, MD, shares his experience with a scleral flap retraction technique to facilitate fashioning of sclerotomy during trabeculectomy. An internal sclerotomy is an essential component of trabeculectomy for glaucoma. Various techniques have been described for proper exposure during this vital and delicate surgical maneuver, however there are many situations where equipment is not available or the surgeon prefers to use knife blade for sclerotomy. Most of the surgeons take help from the assistant acting as a third hand which is cumbersome and carries a risk of scleral flap tear. This video depicts a simple technique used by the author for perfect exposure of scleral bed without taking any help from the assistant.

Thank you for your comments. Placing the suture at limbus makes it independent of ocular movement.
