Excellent video. I had a similar case a few months ago. I chose to explant the one piece acrylic lens through a 3 mm clear corneal incision and proceeded with intrascleral fixation of a three piece lens with an excellent outcome. These cases are relatively rare, and there are many options available (ACIOL, suture fixation as shown here, Gortex fixation of either Akreos or CZ70BD lens, iris claw lens, intrascleral fixation of a three piece lens etc.) Each surgeon should determine what works best in his/her hands and stay with it.
