Reverse Pupillary Block

A myopic woman s/p RD repair with a scleral buckle undergoes phaco IOL. Mild zonular laxity and reverse pupillary block is noted early in the course of surgery. When removing OVD after IOL insertion, an attempt is made to avoid reverse pupillary block which is unsuccessful, resulting in the temporal 180 degrees of zonules being ruptured. The anatomy is restored using a capsule-scleral fixation element. Insights from this case are discussed.
