Michael Patterson, DO, presents a case with repeat IOL dislocations despite a previous scleral fixation. The IOL had dislocated twice in the anterior chamber and now in the posterior chamber. 23G stab incisions are made 180 degrees apart, approximately 1-1.5mm away from the limbus. Sclerotomies farther away from the limbus force the IOL haptics to stretch which makes the lens more susceptible to dislocation.
