In this urgent case of severe primary open-angle glaucoma, with an IOP above 40 mm Hg despite maximum topical and acetazolamide therapy, Sahil Aggarwal, MD, opted to place an Ahmed Glaucoma Valve (New World Medical) for acute IOP reduction. He demonstrates pre-placement of a single suture through the sclera and through the eyelets of the device prior to placing it into the quadrant of choice. This ensures efficiency, improves visibility, and allows for automatic knot burying after the suture is tied off.
