Posterior Polar Cataract Management With a Toric Lens

Posterior polar cataracts can be associated with posterior capsule compromise. In this case, the core nucleus is elevated with hydrodelineation to reduce the pressure against the posterior capsule during phacoemulsification. The cataract is removed in layers, eventually yielding a small posterior capsular opening that did not radialize, allowing for in-the-bag placement of a lens without optic capture.
