This video presents a technique to maintain IOP during implantation of a posterior iris-fixated IOL in aphakic eyes after complicated cataract surgery. Key steps include bilateral 1-mm paracenteses approximately 9 mm apart, pharmacologic miosis, gradual peripheral viscoelastic fill, a 5.5-6.2 mm corneal or scleral-corneal incision according to optic size, retropupillary introduction of the iris-claw lens, and sequential enclavation.
