An inevitable complication of pars plana vitrectomy is occasional damage to the crystalline lens. How do you manage this when it occurs at the very beginning of the case? We show how we managed a large posterior capsular breech created by trocar insertion for PPV. We were able to finish with a 3-piece IOL in the sulcus, with the optic captured in the capsulorrhexis.
