Effectively they are cheap, but may result very expensive if you consider postoperative intraocular hipertension, with it´s serious consequences in neuropaties, more intensive follow up, and meds. Among the many OVD related problems one particularly intringuing is the effect of residual syringue silicone lubricant or the problems related when you try to clean all the silicone micro-bubbles present at the end of the vial. In my case it´s not a matter of savings, we have substituted OVDs for sigle use, the only reusable in my tray is the phaco probe. But most important, no OVD surgeons are so happy worldwide that the common impression is we have been paying massive and unnecesary amounts of dollars in routine cases. Dr. Makcool pointed recently in JRCS to his frustrating early experience with thousands of no OVD surgeries, I think microincision, cold phaco, improved fluidics and injectors deserve a new look to this excinting technique. But, don´t think it´s easy, as always a small change in a surgical step is a mountain even for very experienced surgeons. ¿What do our hindu or african collegues think about this issue? Althought it may seem a paradox, I think It´s time to learn from strategies with no financial interest, no laboratoy driven. I have to make a study, if I were not so lazy, me and my collegues have noted a better wow effect with no OVDs in 24 h check-up.

OVD'S are quite cheap ( specially 2% HPMC ) and has more reasons to be used than just as an AC maintainer..... but the video is excellent and congrats for the same

Good question, benefits are clear, complications also but the risk-benefit ratio is not and millions have been spent in one side of the balance. Having a fixed budget I prefer a complete low cost disposable phaco set insted of OVDs. Of course at times thay are irreplaceable as Dr. Tjia or Chang have beautifully shown here
