Phaco with Retroillumination in Combined Surgery

In this rhegmatogenous retinal detachment case, Nikoloz Labauri, MD, performs phacoemulsification and IOL implantation using chandelier retroillumination instead of microscope light. This is a different way to perform this procedure when the Phacovitrectomy is planned and it carries the advantages of better visualization of the anterior capsule, better assessment of nucleus thickness, safer removal of the cortex, and capsule polishing.

hi how did you calculate the IOL power in the eye which has the detachment.The technique of surgery is excellent.

An IOL calculation on detached retina is always a challenge. Before surgery I usually measure AXL of both eyes using B-Scan echography and the calipers. It is not as accurate as A-scan biometry, but if the AXLs are same in both eyes I take K readings from the detached and AXL (taken by A scan Biometry) from the fellow eye. If B-scan measurements of both eyes are not matched (i.e. unilateral High Myopia) I use only B-scan measurement data and the K reading from the same eye to calculate an IOL Power. Dear Colleagues, Indeed, the brightness which is given with retroillumination is less, but you will be able to see a bit more things which you won’t be able to see with microscope light. Thanking you for your comments
