To achieve the best possible outcome in an eye with a ruptured posterior capsule, our primary goal is to avoid intra- and postoperative vitreous traction. In addition to depicting how traditional maneuvers such as sweeping the anterior chamber to free entrapped vitreous and "Wecking" the incision cause traction, this video shows how to accomplish our goals more safely. Dr. Arbisser demonstrates how to use new instruments to save a descending nucleus without sweeping through vitreous through posterior incisions. The video includes pars plana incision options for anterior vitrectomy and the critical technique of optic capture to leave the patient with a secure implant and optimal result in the face of complications.
