This is a referred case with an ACIOL implanted despite a nucleus drop. The patient was 2 months post-operative and with no improvement in vision. I have used the IOL scaffold technique- an ACIOL scaffold - to emulsify the nuclear fragments after bringing them up via PPV and FAVIT. Having the air pump on during surgery allows a deep enough anterior chamber despite an ACIOL. This is followed by ACIOL explantation and implantation of a glued IOL. I would not however advocate this in case of very large nuclei, hard nuclei, insufficient endothelial count, or a floppy iris that might allow the ACIOL to drop.
