Claudio Trindade, MD, presents a new concept of intraocular implant based on the pinhole effect. It was created to treat irregular corneal astigmatism and pseudophakic presbyopia. The device is a three-haptic implant made of a black hydrophobic acrylic that is designed to be supported in the ciliary sulcus of pseudophakic eyes.

Dr. Ahmed, The device is not commercially available yet. Possibly in 2015. I will keep you updated with the developments. Kind regards. Claudio Trindade

One can examine the retina with infrared equipments, such as OCT, since the implant is 100% transparent to infrared light. (06:20)

Thank you very much for your kind comment. The diameter of the central opening was chosen based on virtual eye modelling performed with the optical engineering software Zemax. We created eye models with different corneal parameters and analysed the wavefront created with different pupil sizes. 1.5mm was the number that seems to create the best balance between depth of focus and retinal luminance. A second stage of testing was done in an optical bench. We could see remarkable neutralisation of aberrations by incorporating the implant into an "aberrated" laser beam path. We tested different sizes and again 1.5mm had the best results. Regarding centration, the 3-closed loop haptic, allows better vector force distribution and better centration in the sulcus. The hydrophobic acrylic is stiffer than hydrophilic, which gives more resistance to the haptics, with less deformation and descentration. However sulcus anatomy vary from patient to patient, and UBM examination prior to surgery may provide important data (sulcus diameter, pupil position in relation to ciliary sulcus center point). Since ciliary sulcus is not very rigid, the implant is not 100% ”locked" in place, and, during surgery, small adjustments in centration can be obtained by simply moving it with a hook. During surgery the implant is sensitive to fluidics, and may move during OVD removal, especially if irrigation is high, because of sulcus expansion. We have been measuring centration, and so far, only slight decentrations have been observed. Kind regards, Claudio Trindade

A great invention! I think this will change our approximation to presbyopia in the near future. I wonder how you defined the proper size hole in the center of the implant? There is some degree of critical decentered to this implant in pseudophakic human eye that you have been measured? Congratulations on your excellent work.

How do you view the retina through this?
