miLoop in brown cataract with posterior plaque

Bennett Walton, MD, MBA, shows a brunescent lens in a patient with preoperative hand motion vision that is efficiently bisected with the miLoop device (IanTech). The miLoop is again used to bisect a heminucleus. A posterior plaque is noted upon nucleus removal but is too adherent to be removed with irrigation or aspiration. Once the IOL is in the capsule, Utrata forceps are used for attempted plaque removal, but it is too adherent and will be addressed with a YAG capsulotomy post operatively.

Very nice surgery. The endothelium can suffer too much with high and prolonged use of ultrasound. I think this patient can do safer with a extracapsular surgery or sics. Congrats on the technique.
