This patient presented with a stage 4 full-thickness macular hole. In this video, surgeons demonstrate a method for internal limiting membrane (ILM) peeling for macular hole repair in which an ILM splitter is used to extend the ILM flap. The ILM is peeled toward the center of the macular hole to release any attachments at the margin of the hole.
