Altan A. Ozcan, MD, shares a case of a 51-year-old man with cataract and congenital iris-lens coloboma in both eyes. Cataract surgery with CTR and iris reconstruction was performed under peribulbar anesthesia. Phacoemulsification with cosmetic repair of the coloboma can be a useful and safe procedure in cataract patients with congenital iris and lens coloboma.

The thing we are trying to tell ; it is not clear where the pupil should be.. Therefore we did not want to shift it up so much. Otherwise full centering is not an issue.

Thanks for the interest and comments. But we did not aim to make the pupil more centered because of concerns related with neuroadaptation to the visual axis he had before.

i think you are mistaken. even with iris coloboma, plenty of light is entering at the center of the eye, aligning with the fovea. with the pupil more centered and round, all that is done is limit the peripheral light- the visual axis remains the same. there is no neuroadaptation. even if the center of the new pupil is shifted by a mm, the recti muscles can easily shift a few degrees to keep the light on the fovea. i am not aware of any issues in those surgical procedures in these coloboma cases where the pupil is better centered.

i saw great video here on eyetube about iris coloboma repair. one gets better results if one cuts the sphincter and suture them together more proximaly, and then suture the more distal iris-the pupils becomes more centered.
