In this case, the anterior capsule is snagged during removal of the last bit of cortical material. The single piece acrylic IOL was already in the capsular bag. The 90 degree I/A tip was used to remove a bit of residual cortical material. Unfortunately the anterior capsule was tugged along with the cortical material which led to a significant zonular dialysis in the subincisional area. As several clock hours of zonules appeared to be damaged it did not seem likely that a simple capsular tension ring (CTR) would work. The surgical team decided to suture a capsular tension segment (CTS) to the sclera with a sliding internal knot ala Siepser.

even pre-placed suture could be passed with the help of 26G needle as a guide and knot could be rotated after tying. there is some residual loosening even after maximum tightening in seipser. as whatever you do, limbus can't be pulled in or knot can't slide upto sulcus. loop may be pulled from outside and tightened. but this much AC manipulation could be avoided? of course, it's always safe to keep a flap over the suture rather than keeping it subconj or exposed.

is the external portion of the suture pass on the sclera covered by conj only? Any issues with erosion so close to the limbus and not covered adequately? thanks for video.
