Iqbal Ike K. Ahmed, MD, shares a case of dislocated IOL exchange with a 3-piece hydrophobic IOL sutured to posterior iris. He outlines several benefits of this approach versus a scleral-fixated IOL, including no externalization of haptics/tract, less tilt, and minimal vitrectomy requirement. He says it also maintains the IOL in a more natural position and away from the cornea compared to an ACIOL.
