Parnian Arjmand, MD, presents a case of a large 533-micron chronic macular hole with pigmentary RPE changes. Posterior staphlyoma and atrophy in the posterior pole are noted. Access is facilitated using a diamond-dusted scraper and anatomical success is maximized using an inverted ILM flap technique attached to the edge of the posterior staphyloma.
