Tal Raviv, MD, presents the management of a 4-5+ leathery cataract. In mature cataracts, Dr. Raviv prefers a large capsulorhexis and a gentle hydrodissection. He uses a combination of horizontal and vertical chopping maneuvers to debulk the lens but it resists fracture. After viscoelevating one of the quadrants, he converts to an internal extracapsular technique.

fantastic technique for an incredibly difficult case.Dr Raviv what are your flow and vacuum settings and do you generally advise fixed or linear flow/vacuum settings for routine cases? thanks.

these cases make me miss the Mackool 1.1 flare. Not only was it fast, but there was negligible chance of corneal burn.

An alternative method that i like calling the double nucleus technique; is to substantially debulk the nucleus with the bevel of the phaco tip looking down. After doing so, one can deal with the remainder of the nucleus as one would in a normal (less dense) nucleus, preferably by grooving and cracking the the remainder of the nucleus.

thats a good supracap technique. i also advise chang chopper- very, very sharp, and can vertical chop very dense lens.
