This patient had success with an implantable collamer lens (ICL) for 15 years. In the last 2 years, he had complained of decreased vision in his right eye. An anterior cataract was forming and interfering with his vision. Cataract surgery was elected, and during surgery it was noted that the ICL was loose and moved freely in the sulcus position.
