Omar Barrada, MD, demonstrates how to successfully perform a spiral rhexis. A very useful technique when attempting a capsulorhexis in intumescent and mature cataracts to help prevent the occurrence anterior capsular tears and the Argentinian flag sign.

good case but i think still unsafe to have a leading tear with high intralenticular pressure,it`s better to finish even a very small rhexis, aspirate liquid cortex ,add more visco elatic, here you can proceed safely my point is that spiral way wouldn`t prevent extension if intralenticular pressure is high

good case but ? think it is better grasping the capsul from proximal to minimise the radial tears.
